End of Industrial Attachment Documents and Paperwork
There are several documents and paperwork that you will need to have completed by the end of your Industrial Attachment.
Instructions: After reading the checklists, you can copy them to your notebook, where you will keep track of their completion. Don't forget to bring your notebook to your last days of IA so that you remember to complete your final tasks and needed documents!
Below is a checklist of the paperwork and documents that may be required:

□ Weekly Reporting Sheet
□ Self-Evaluation Sheet
□ Student Logbook
□ Company Instructor/Supervisor Logbook
□ Recommendation Letter
□ Industrial Attachment Report (if applicable)
Note: This list does not contain all the documents you might need, and those mentioned might not apply to everyone. Please ensure that you ask your school supervisor and company about any other documents you are required to have completed and add them to the list.
